This provides us with essential background history, previous concerns, and can allow us to continue ongoing care for existing conditions. If you would like us to obtain history from more than one clinic, please list all relevant clinic names on the form.

If you are moving to a new clinic:

While we are sad to see you go, we would like to ensure your transition to your new clinic is as smooth as possible. Completing this form allows us to send your pets history from our clinic to your new veterinarian. Please note that we are not authorized to send copies of your pets history from other veterinary clinics you have previously been to.
