Submitted refills should be available for pickup 24 hours after submission.

Some specialty medications may require longer order times. Please order refills 2 to 3 days before running out to ensure you don’t run out.

Pre-Surgical Intake Form

Please submit this form prior to your pet’s scheduled surgery drop-off time. This allows us to know all of the necessary information before your pet arrives with us so that we can ensure surgery is as safe and comfortable as possible.

A technician will go through the information you have provided with you at the time of drop off to confirm there have been no changes.

Please expect to spend 5-10 minutes with our team to help make your pets transition into our care as smooth as possible.

Medical Record Transfer Request

If you are a new client to us:

Completing this form allows us to reach out to your pets previous veterinary clinic and obtain a copy of their medical history. This provides us with essential background history, previous concerns, and can allow us to continue ongoing care for existing conditions. If you would like us to obtain history from more than one clinic, please list all relevant clinic names on the form.

 

If you are moving to a new clinic:

While we are sad to see you go, we would like to ensure your transition to your new clinic is as smooth as possible. Completing this form allows us to send your pets history from our clinic to your new veterinarian. Please note that we are not authorized to send copies of your pets history from other veterinary clinics you have previously been to.
