Please submit one request form per pet. Once we receive your request, a member of our team will call you to schedule an appointment.

If this is an emergency, please call the clinic directly at 780 737 7297. If it is outside of our business hours, please seek assistance from your closest emergency vet.

If this is your first visit to our clinic, please complete a new client registration form prior to your scheduled appointment date. Your prompt submission of this form allows us to set up your client profile more efficiently and provide your veterinarian with your pets information before your appointment.
